How We Define Success

  1. Permanent Housing Transition
    Target: 75–85% of residents transition to permanent housing
    Success means residents move into sustainable, independent housing — not return to homelessness or crisis systems.
  2. Financial Readiness & Stability
    We track:
    • Earned income changes
    • Benefits enrollment stabilization
    • Participation in workforce pathways
    • Progress within our Housing Readiness Savings Program
  3. Long-Term Retention
    Housing stability 6–12+ months after exit
    Long-term success is measured by whether residents remain housed after completing the program.
  4. Reduced Returns to Crisis Systems
    We aim to reduce participants’ reliance on:
    • Emergency shelters
    • Hospital readmissions
    • High-cost crisis response systems
  5. Goal Completion Rate
    We measure residents’ progress toward individualized stability plans that ensures progress toward independence:
    • Employment or income goals
    • Benefits or resource milestones
    • Life-skills benchmarks
    • Housing search and placement achievements
